Thank you for your interest in booking with us! This is the form you will fill out if you are interested in booking with Alex Cook, Tyler Blumenthal, Bradley Clark, Jp Gomez, Tim Cook, Cassy Grymm, Calloway, or Sam Strom.

By filling out this form you agree to the terms provided in the info below.

We require a $100 (plus tax) deposit that is NON-REFUNDABLE. This deposit does go towards the cost of the tattoo, holds your spot in our books, and ensures us that you will come to the appointment.

Numbing cream is something that is allowed (with all artists aside from Bradley) but we DO NOT provide. If you opt to use numbing cream we need to know BEFORE the appointment and cannot be above 4%. If you use numbing cream, you consent that it will affect the end result of your tattoo.

• Washington state requires sales tax on all items and services, including tattoos. As a legal business, we add sales tax to the final cost. For instance, if you are quoted 2 hours at $150 your total will come out to 324.60.

Cash is preferred, as we have a 3% charge on cards.
